BREAKING: The NTSB has released its preliminary findings in the crash of UPS flight 2976, an MD-11F aircraft with tail number N259UP, at Louisville's Muhammad Ali International Airport on November 4. The aircraft was departing from runway 17R at 17:14 Eastern time. The subsequent crash killed 3 onboard crew and 11 on the ground, and injured 23 people. The plane was bound for Daniel K. Inouye International Airport (HNL) in Honolulu, Hawaii..
PRELIMINARY FINDINGS:
β Mechanism VERY similar to American Flight 191, whose left engine separated on May 25, 1979 while departing O'Hare; 273 people were killed
β Airport surveillance video of the airplane showed the left (No. 1) engine and pylon separating from the wing shortly after airplane rotation
β A fire ignited on the left engine as it traversed above the fuselage and impacted the ground; a fire also ignited near the area of the left pylon attachment
β The airplane initially climbed but did not get higher than about 30 ft above ground level
β The airplane cleared the blast fence beyond the end of runway 17R, but the left main landing gear impacted the roof of a UPS Supply Chain Solutions warehouse at the southern edge of the airport
β The airplane then impacted a storage yard and two additional buildings, including a petroleum recycling facility, and was mostly consumed by fire
β A witness in the air traffic control tower reported that the takeoff speed appeared normal for that type of aircraft; however, the climb rate was not normal, as the airplane did not climb above the tower's height of approximately 200 feet
β Another witness reported that the airplane stopped climbing and began to lose altitude before rolling slightly to the left
β The left engine pylon aft mount's forward and aft lugs were both found fractured near their 2 o'clock (inboard fracture) and 9 o'clock (outboard fracture) positions when in the aft-looking-forward frame of reference
β The fractured and separated upper portions of the forward and aft lugs were found adjacent to runway 17R
β At the time of the accident, N259UP had accumulated a total time of about 92,992 hours and 21,043 cycles
β A 24-month/4,800 hour lubrication task of the pylon thrust links and pylon spherical bearings was last accomplished on October 18, 2025

WICHITA, Kan. β A Wichita police officer was seriously injured Thursday afternoon while attempting to take a suspect into custody during a child abuse investigation.
Around 4:30 p.m., officers responded to a residence in the 1000 block of 54th Street South as part of a follow-up on a child abuse case. Upon arrival, they contacted a 33-year-old man at the home.
The man immediately became aggressive and punched one of the officers in the head. As officers attempted to detain him, they called for additional units. During the struggle, the suspect struck the officer multiple times in the head before he was eventually subdued and taken into custody.
The injured officer was transported to a local hospital with serious injuries but is expected to recover.
